Aight, actually looked around and saw there was a proper format for this, my bad!

Survival or Creative: Global
Admins online during the incident: _pancakes_ for one, not sure about any others.
What ban reason do you receive when attempting to log on?: XRay modding
Why do you believe you should be un-banned?: Well it's been a while and I would like to rejoin the server fresh and no longer dirtied with cheats.
Aus-Craft was a really friendly place and it would be cool if I could have a second chance.

Edit: Weird, mcbans says I have no bans on record but the link given to me on the login screen clearly shows it.
Not sure what to do here.

Sorry it took so long to get back to you on this.
As _pancakes_ is no longer a part of the staff here, I've come to handle this for you. Given the length of time it has been since your ban, it has been decided that we are willing to give you the benefit of the doubt and give you a chance to prove you have changed your way. Therefore, we will allow you back onto the server. Understand, of course, any more breaches of the rules will not be tolerated.
